The form can be completed, signed and sent directly to the company to which the electronic payments are being made or alternatively you can send the form to a Wide Bay Australia's Administration Department at Reply Paid 1063, Bundaberg QLD 4670; or alternatively, fax through to (07) 4152 3499. Wide Bay Australia has no control over when the organisation making the debit to your Wide Bay Australia account will update their records.
